The civil marriage application form is JPN.KC02 (Marriage Application Form and Statutory Declaration), not JPN.KC05 (JPN Civil Marriage Registration page).

JPN’s fees are RM10 for the marriage notice and RM20 for the marriage certificate on the ceremony day — not RM30 plus RM20–30 (“about RM91”). The application is valid for 6 months from the date you submit it, not 6 months after the 21-day notice (same JPN page).

The HIV test is not required for everyone. JPN’s document list for civil (non-Muslim) marriage does not include it. MOH says the Cabinet made pre-marital HIV screening compulsory for Muslim couples from 2009 and only encouraged it for other religions, and JAKIM’s guideline requires Muslim couples’ results to be verified by a government medical officer (MOH/JKN Johor briefing, JAKIM marriage guideline).

Non-Muslim couples who choose to test can do it at a Klinik Kesihatan too.

The Procedure:

  1. Check whether you need an appointment. The Malaysian Government portal says to make an appointment through the MySejahtera app, while JKN Selangor (the Selangor State Health Department) says pre-marital screening does not need an appointment in advance. Practice may differ between states, so confirm with your local Klinik Kesihatan before you go.
  2. Tell the counter: “Nak buat HIV test untuk kahwin”.
  3. Pay the RM 1.00 registration fee. MOH says the HIV test itself is free for Malaysian citizens.
  4. They prick your finger.
  5. Wait 15 minutes.
  6. Get the result slip. DO NOT OPEN THE ENVELOPE if they seal it! (Though nowadays usually they just give you the form).

JPN.KC02 is the Marriage Application Form and Statutory Declaration. JPN accepts only original forms, which you get at any NRD (JPN) office. JPN also takes civil marriage applications online at oskc.jpn.gov.my.

2. The Statutory Declaration

The form includes a statutory declaration about your marital status. JPN’s current service page does not list a separate Commissioner for Oaths step, so ask your NRD branch how the declaration must be made before you go.

3. Submission (The 21 Days Rule)

Submit at the NRD office for the address on your MyKad. JPN’s checklist for Malaysian citizens: MyKad (original and copy), your latest birth certificate extract if you never had an identity card (original and copy), and one passport-size colour photo (blue or white background) for each applicant.

  • Fee: RM 10.00 (marriage notice).
  • The Wait: Your application will be displayed for 21+1 days. This is to allow anyone to object (e.g., your secret ex-wife).
  • Validity: The application is valid for 6 months from the date you submit it.

4. The Big Day (Solemnization)

Book a date. Bring 2 witnesses (friends/family aged 21+). Dress properly (No jeans/slippers!).

Path B: Muslim (Nikah / Daftar Kahwin Online)

For Muslims, Register Marriage Malaysia 2026 is handled by the State Religious Department (JAIS/JAWI, etc.).

1. Kursus Kahwin (Marriage Course)

Mandatory. It takes 1.5 to 2 days.
